Who is this test for?
Recently, the American Association of Clinical Endocrinology (AACE) indicated that clinicians should consider persons with the following conditions to be at high risk for NAFLD and advanced fibrosis:
- Obesity
- Metabolic syndrome
- Prediabetes
- Type 2 Diabetes. Should consider screening using FIB-4, even with normal liver enzyme levels
- Hepatic steatosis on any imaging study
- Persistently elevated ALT or AST levels (over 6 months)
The AACE recommends ordering FIB-4 index as the preferred test for the screening for clinically significant fibrosis in NAFLD patients. Clinicians should consider further follow-up with transient elastography or ELF blood test in persons with increased risk of advanced fibrosis and indeterminate or high FIB-4 score.
